Carnipure by Meditech AKA Acetyl-L-carnitine or ALCAR, is an acetylated derivative of L-carnitine. It is a fantastic dietary supplement and occurs naturally in plants and animals. During strenuous exercise, as is the case with athletes and bodybuilders, a large portion of L-carnitine and unused acetyl-CoA are converted to ALCAR and CoA inside mitochondria of the body cell by carnitine O-acetyltransferase. The ALCAR is then transported outside the mitochondria with the help of transportation cells, where it converts back to the two constituents. The L-carnitine is now cycled back into the mitochondria with acyl groups to enhance fatty acid utilization.

It has been shown in laboratory studies that Carnipure is far superior to L-carnitine in terms of bioavailability. Both use the same mechanism for absorption in intestine that gets improved with sodium. One study has shown that Acetyl L-carnitine has a lower blood concentration after ingestion. This might be because Acetyl L-carnitine is hydrolyzed more in blood. This derives that it has less bioavailability unless it is entering cells (e.g. muscle) more efficiently from the blood than L-carnitine.
